We report, in this dissertation, four cases related to patients admitted to the emergency department of the University Hospital Center of Béjaia for management of hemorrhagic shock, and this to encourage strengthening the appropriation of the 2015 recommendations of the French Society of Anesthesia and Resuscitation. Diagnosis of hemorrhagic shock is easy in case of exteriorized or extrene hemorrhage and is difficult in case of internal hemorrhage. In the initial phase of hemorrhagic shock, it is essential to avoid worsening the bleeding, by diluting the coagulation factors, as long as the bleeding is not controlled. After having started the vascular filling, it is probably necessary to administer a vasopressor in the event of persistence of arterial hypotension (PAS<80 mm Hg). It is recommended to have a standardized massive transfusion strategy to correct coagulation disorders as quickly as possible. In the event of haemorrhagic shock occurring in a patient treated with vitamin k antagonists, it is recommended to administer prothrombin complex concentrates without delay. Management of hemorrhagic shock is a multidisciplinary approach.
Hemorrhagic shock is an emergency whose delay in treatment leads to excess mortality. Rapid identification of the mechanisms involved; the etiology of the state of shock and its severity can guide its management. This must be based on precise protocols resulting from the recommendations. | | Note de contenu : | Mémoire de fin d’étude en vue de l’obtention du Diplôme de docteur en sciences médicales |
