This retrospective and descriptive study was conducted in the Department of Neurosurgery at Khalil Amrane University Hospital in Bejaia, Algeria, covering the period from January 2019 to March 2025. The study aimed to establish the epidemiological profile, clinical presentation, diagnostic modalities, and therapeutic strategies of CPA tumors in the local patient population.
A total of 34 patients diagnosed and treated for CPA tumors were included. Data were collected from medical records, surgical protocols, and imaging studies, then analyzed statistically. Vestibular schwannomas were the most frequent tumor type (58.8%, 20 cases), followed by epidermoid cysts (17.6%), arachnoid cysts (8.8%), meningiomas (5.8%), and schwannomas of mixed nerves (5.8%), with
a single case of glioblastoma (2.9%). The incidence of CPA tumors showed a marked increase after 2023. Clinically, the most common presentations were hearing loss, dizziness, tinnitus, and cranial nerve deficits, occasionally accompanied by signs of intracranial hypertension and cerebellar dysfunction.
Magnetic resonance imaging (MRI) was the cornerstone of diagnosis and preoperative planning, while functional tests such as audiometry retained an essential complementary role. Surgical treatment remained the primary therapeutic modality, with the retrosigmoid approach most frequently employed. The extent of resection was often constrained by tumor size and adherence to adjacent neurovascular
structures, particularly in advanced Koos grade tumors. Subtotal or partial resections were adopted in selected cases to preserve neurological function. Postoperative complications included cerebrospinal fluid leaks, cranial nerve deficits, and, rarely, hydrocephalus or infections.
In conclusion, CPA tumors in Bejaia mirror international epidemiological trends, with vestibular schwannomas being predominant. Optimal management requires a balance between maximal safe resection and preservation of neurological function, with individualized treatment strategies guided by tumor type, size, and patient condition. Advances in imaging and surgical techniques continue to improve diagnostic accuracy and therapeutic outcomes. | | Note de contenu : | Mémoire de fin d'étude en vue de l'obtention du diplôme de docteur en sciences médicales |
